Read the General Information below. When you understand and agree to these terms, submit the completed form to DSF. Once it is processed, you will receive a confirmation rental fee notice (if applicable) for your reservation from DSF staff.

NOTE: Although it is most unlikely that you will experience any problems responding to this form, certain non-standard browsers will not respond properly. If you experience any difficulties, (or if you are not using a forms-capable browser) you may email your response to this form to: info@dsf.edu. Schedule:  single programs are rented for one week; series, for one week per segment.  All videotapes are to be returned on the first working day after the final showing.

Cost:  as listed, plus first class postage (one way to renter).  First class return postage is additional responsibility of renter.  Rates:  one or two tapes, $3.00; three tapes, $4.00; four tapes, $5.00.  An invoice will be sent at the time the tapes are mailed, payable at the conclusion of tape use.

Responsibility:  The renter or borrower is responsible for the videotapes between delivery and return, including any damage incurred while viewing or loss en route to the Seminary Foundation.

Support materials:  Leader's guides accompany most of the rental tapes.  Posters for advance publicity are sent with most titles.
